Annual Title I Needs Assessment Survey 

 

Instructions:  Click on the link above to complete our survey.  Your response is anonymous unless you choose to add your name or email address at the end of the survey.  Thank you for taking the time to complete our survey.  Your feedback is important in helping shape our Title I program and parent involvement activities. 

Any parent who needs to use a computer to complete the survey can stop by the Front Office on Monday, Thursday, or Friday of this week. We ask that you please not stop by during testing days and times (Tuesday or Wednesday). Thank you!

and select Three Oaks Middle School and our school
